For appointments please call our office during business hours at (973) 292-0999.
 
Please be prepared to provide us with the patient's name, address, phone number, birth date, type of insurance, reason for appointment, and referring physician, if applicable.
 
The best time to call for an appointment is between 9:00 a.m. and 4:00 p.m.

If you are a new patient to our practice, your initial examination, depending on the complexity of the problem, will take approximately one hour. Please try to arrive 15 minutes early to complete a patient registration form. Follow-up examinations are usually scheduled for 15 minutes to one-half hour depending on the nature of the problem and the length of time since the last visit.

We try our best to stay on schedule, although emergencies sometimes arise. If we are seriously delayed, we will try to notify you beforehand, if possible. We ask your patience if you have to wait; we know your time is valuable.

What to Bring to the Appointment:

Patient's insurance card
A list of current prescriptions and/or
over-the-counter medications you

Physician referral forms,if required
iiiby insurance

Relevant information about your medical
and surgical history, copies of all relevant
iiitest results, recent x-rays and
iiidoctors' notes

Appointment Cancellation
If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to be of service to other patients who are in need of our care.

Payment Information
Payment for services rendered is the responsibility of the patient, regardless of insurance coverage. Depending upon the individual contract that exists between the patient and insurance carrier, reimbursement may be subject to policy deductibles and/or percentage of allowable charges. We are happy to assist you with information your insurance company may require to process your claim.

Prescription Renewal
If you need renewals of your current prescriptions, please call the office during normal working hours at least one week in advance. Please do not wait until you are out of medication before calling. Emergency requests for prescription refills after hours and on weekends will be limited to a one-week supply.
